You will need a referral from your General Practitioner. This may be faxed or emailed to us prior to your consultation; alternatively, it can be brought with you. You may still be able to attend without it, but you will not be eligible for a Medicare rebate.
Please arrive 5-10 minutes earlier than your scheduled appointment time as you will need to complete patient registration forms. Generally, you will receive an email with the forms attached. You may choose to fill these out prior to your appointment.
The number of available parking spaces on the street and in nearby car parks is restricted. We advise allowing sufficient time to park at Campbelltown Public Hospital, which is conveniently located within a brief, 5-minute walking distance from our office. The parking fee at this location is $10 for a duration of 2 hours.

Kindly find the PDF link to access our patient registration forms. If your appointment is with Dr Stewart concerning a breast-related issue, please ensure to fill out the Initial Breast Forms. For all other matters with Dr Stewart, kindly complete the Initial General Forms. You have the option to print and fill out these forms in advance to bring them with you on the day of your consultation.
· All patients must have a current GP referral for their visit to qualify for a Medicare rebate

· Urgent breast patients will be seen within a day or two
Initial Consultation:
· $250 (approximately $80 Medicare rebate)
· Aged pensioners: bulk billed
Annual Reviews/Returning Patients >12 Months:
· $250 (approximately $80 Medicare rebate)
· Aged pensioners: bulk billed
<12 Month Reviews:
· $150 (approximately $40 Medicare rebate)
· Aged pensioners: bulk billed
Cancellation Policy:
You do not need to pay a deposit to make an appointment with Dr Stewart however, if you cancel your appointment within less then 24 hours or do not attend a scheduled appointment, you will be charged a non-refundable deposit of $100 to make a new appointment.
Our commitment to you:
We appreciate that your time is valuable. Dr Stewart’s schedule can be interrupted by unforeseen emergency consultations and operations at Campbelltown Hospital. In this situation, we may unfortunately need to unexpectedly reschedule your appointment at short notice. If we reschedule your appointment with less than 48 hours’ notice, we will make every effort to reschedule your appointment within two weeks.
